Practice Policies & Required Documents
Before beginning services, clients are asked to review and acknowledge a few important documents that outline how therapy works, how your privacy is protected, and what to expect from care.
These include:

Describes the nature of therapy, potential risks and benefits, client rights and responsibilities, fees, scheduling, and professional boundaries.

Explains how your protected health information (PHI) may be used or disclosed, and outlines your rights regarding your records.

Provides information about working with an associate therapist, including supervision requirements and how clinical oversight supports quality care.

Your experience matters. If at any point you have questions, concerns, or feedback about your care, you are encouraged to speak openly with me about them. Many concerns can be addressed directly and productively within the therapeutic relationship.
I take all concerns seriously and aim to respond with care, transparency, and professionalism.

If a concern cannot be resolved through the steps above, you have the right to file a formal complaint with the licensing board that regulates mental health professionals in Texas. Clients are encouraged (but not required) to attempt to resolve concerns with the clinician and/or supervisor prior to filing a complaint with the board.

This board oversees the licensure and regulation of mental health professionals in Texas, including Licensed Professional Counselors.You may call 1-800-821-3205 to request the appropriate form or obtain more information (please note, this phone number is for complaints only.)
